Little changes to big results. That's the goal Abbott and Omada Health want to deliver as part of their new diabetes care collaboration.

Integrating the revolutionary FreeStyle Libre system, a continuous glucose monitoring (CGM) technology, with Omada Health's pioneering digital coaching platform, the companies aim to help people living with Type 2 diabetes tackle the challenges of managing their condition with personalized, on-the-go care and community support.

Diabetes Care: Small Change, Big Result

Here's the game plan:

Who’s eligible? With more than 30 million Americans with diabetes — 95% of whom are living with type 2 diabetes — many are looking for simple ways to better manage their condition and overall health. If you have access to the Omada Program through your health plan, employer or health system, you can get on the roster.

How do I get in the game? Omada Health will offer the integrated solution to employers and health plans. People diagnosed with Type 2 diabetes who are eligible will have an online physician consultation to obtain a prescription of the FreeStyle Libre system.

Then what? A welcome kit will be delivered to your doorstep, including a wireless scale and a FreeStyle Libre system.

The integrated platform, powered by the companies' proprietary algorithms and analytics, combines:

  • FreeStyle Libre's one-second scans for real-time glucose readings every minute, current glucose trends and historical trends and patterns with …
  • Omada Health's leading platform to provide actionable information that supports you as you better manage your diabetes, including seamless data tracking.

That’s the playbook.

Got it. Coaching? When you’re up and running with Omada Health's proprietary digital coaching platform, you’ll receive tailored recommendations from certified diabetes educator (CDE) coaches through online chats in the Omada app.

You’ll also have access to interactive educational lessons, support from an online peer group and tailored action plans to help build long-term healthy habits to achieve your goals.

The integrated program from Abbott and Omada Health will help people living with Type 2 diabetes reach better health to live fuller lives.
